Ontario company fined $130k following a fatal health and safety incident at one of their facilities.

Section 25(2)(h) of the Occupational Health and Safety Act requires an employer to take every precaution reasonable in the circumstances for the protection of a worker. The investigation determined that a reasonable precaution would have been to ensure control switches or mechanisms for the machine in question were locked out to prevent the starting, where the starting could endanger a worker.
